The subscription fee is automatically charged from your provided card at the end of the subscription fee period. This applies to Model 1 and Model 2 Evaluation accounts.
The subscription on your account will not be automatically canceled if you break a rule. If you violate a rule like EOD Trailing Drawdown, that account will be closed, but it will not change the subscription billing date or cancel it.
During the next billing period:
1) When the evaluation account is failed at the time of the next billing period, it will create a new account based on the order with a subscription. The subscription is then extended by another 30 days.
2) If your account is still active and you did not breach any rules, the progress will remain intact. The subscription is then extended by another 30 days.

The monthly subscription on the Live Funded account
Since the Live Funded account is a real account created at a brokerage with actual funds the main difference is that the monthly subscription is deducted from your balance, not as a payment from your card.
